Prism

Ask

You take care of patients.

Your policy software should take care of itself, and take care of you (and your entire organization).

Designed by practicing healthcare quality and regulatory leaders who have managed the same surveys, policy libraries, and corrective-action work.

Where the current process breaks

The policy problem usually becomes visible at the worst possible time.

“Show me the policy.”

A surveyor asks for proof. The answer exists—but three people search different folders.

“We have 14 days.”

Trace the requirement, update the policy, route approval, educate staff and prove completion.

“I inherited 39 policies I didn’t write.”

Which are stale, conflicting, or missing requirements? Now they’re mine to fix.

“Which one is the real policy?”

Two policies govern the same workflow but disagree. Version control can’t tell you which is right.

Your policy repository stores documents.

Why we built Prism

Built by healthcare leaders who have had to prove the work.

Prism was designed by practicing physicians, chief quality officers, health-system executives, and healthcare technology leaders who have been responsible for regulatory readiness, accreditation surveys, policy governance, and clinical operations themselves.

We built the regulatory and policy intelligence we wished we’d had.

  1. Built by insiders

Created by people who have had to prove coverage at survey — not software people guessing at compliance.

  2. AI inside your governance

Understands every document against every applicable obligation, within your approval process.

  3. Outcome, owned by you

Source-traced and human-approved. Nothing publishes automatically.

Know

Compliance intelligence:your library, made intelligent.

Prism maps each policy to CMS, accreditor, and state obligations—then shows what is covered, current, consistent, and contextual.

  • Analyze your policies, every finding tied to a source

  • Evaluate consistency, flagged for mismatch and change

  • Know the exact path from policy to obligation

  • Avoid costly surprise gaps backed by traceable evidence

Use

Answers at the pace medicine actually moves.

Can someone document in the medical record on behalf of a provider?

Sources checked

Thought for 1s

Sometimes, but not as if you were the provider.

•When someone other than the actual care provider documents, the provider’s name should appear in the entry. Student documentation must also be confirmed by the responsible RN or clinical instructor. [1]
•Non-radiologists may not sign or electronically sign radiology reports. [2]

The entry should identify both the provider and the person documenting.

Verbal orders accepted by RNs or clinic MAs must be written according to approved guidelines for orders.

4.

When documentation is done by someone other than the actual care provider, the provider’s name should appear in the entry. Student documentation must be confirmed by the RN responsible for that patient’s care or the Clinical Instructor.

5.

Only hospital approved abbreviations and forms are to be used for documentation in EMR.

Policy authors start from the gap, not a blank page. Prism drafts cited language in hospital context; your team edits, routes, and owns it.

  • Draft new policy language from selected gaps and cited obligations

  • Rewrite existing policies for consistency and current compliance

  • Route edits through your own review and approval process

  • Keep every suggested change connected to the evidence trail
