Basic requirements. Participation as an REH is limited to facilities that—
(a)
Meet the definition in § 485.502.
(b)
Have in effect a provider agreement as defined at § 489.3 of this chapter to provide services.
(c)
Meet the conditions of participation set out in this subpart.
