Restraint and Seclusion

The hospital uses restraint or seclusion only when it can be clinically justified or when warranted by patient behavior that threatens the physical safety of the patient, staff, or others.

Use of restraint or seclusion is in accordance with a written modification to the patient's plan of care.

Use of restraint or seclusion requires an order from a licensed practitioner permitted by the hospital and within the state-law scope of practice.

The hospital monitors patients who are restrained or secluded.

The hospital has written policies and procedures that guide the use of restraint or seclusion.

The hospital evaluates the effectiveness of its restraint and seclusion practices.

The hospital documents the use of restraint or seclusion.

(15)

The hospital reports deaths associated with restraint or seclusion.

Staff are trained and competent to minimize the use of restraint or seclusion and, when use is indicated, to use restraint or seclusion safely.

(19)

Medical staff and licensed practitioners who are authorized to order restraint or seclusion are trained on the use of restraint and seclusion.
